Dosing & Administration
Adult Dose
650-1000 mg PO/IV every 4-6 hours (Max 4g/day).
Pediatric Dose
10-15 mg/kg PO/IV every 4-6 hours (Max 75 mg/kg/day).
Renal Adjustment
CrCl 10-50: every 6 hrs. CrCl < 10: every 8 hrs.
Hepatic Adjustment
Use with caution in severe hepatic impairment. Max 2g/day.
Safety & Special Populations
Pregnancy Category
B - Generally considered safe.
Lactation Safety
Compatible. Excreted in breast milk in small amounts.
