Dosing & Administration
Adult Dose
500-1000 mg PO every 8-12 hours.
Pediatric Dose
40-90 mg/kg/day PO divided every 8-12 hours.
Renal Adjustment
CrCl 10-30: 250-500 mg every 12 hrs. CrCl < 10: 250-500 mg every 24 hrs.
Hepatic Adjustment
No adjustment necessary.
