Dosing & Administration
Adult Dose
DVT/PE: 10 mg BID x7 days, then 5 mg BID. A-Fib: 5 mg BID.
Pediatric Dose
Not recommended.
Renal Adjustment
A-Fib dose reduced to 2.5 mg BID if 2 of 3: Age ≥80, Weight ≤60 kg, SCr ≥1.5.
Hepatic Adjustment
Avoid in severe impairment (Child-Pugh C).

Pregnancy Category
C - Avoid if possible.
Lactation Safety
Probably incompatible.

- Mechanical heart valves
