Dosing & Administration
Adult Dose
250-750 mg PO BID or 400 mg IV every 8-12 hours.
Pediatric Dose
10-20 mg/kg PO BID (use with caution).
Renal Adjustment
CrCl 30-50: 250-500 mg every 12h. CrCl < 30: every 18-24h.
Hepatic Adjustment
No adjustment necessary.
Safety & Special Populations
Pregnancy Category
C - Avoid if possible (cartilage toxicity).
Lactation Safety
Usually compatible (avoid nursing for 3-4h after dose).
Contraindications & Adverse Effects
Contraindications
- Concurrent tizanidine use
Common Adverse Effects
- Tendon rupture
- QT prolongation
- C. diff infection
- Peripheral neuropathy
