Dosing & Administration
Adult Dose
20-80 mg PO/IV daily or BID.
Pediatric Dose
1-2 mg/kg PO/IV every 6-12 hours.
Renal Adjustment
Higher doses required in severe renal impairment.
Hepatic Adjustment
Monitor closely for electrolyte shifts leading to hepatic encephalopathy.
