Dosing & Administration
Adult Dose
500 mg PO/IV every 8 hours.
Pediatric Dose
30-50 mg/kg/day PO divided every 8 hours.
Renal Adjustment
No adjustment unless CrCl < 10.
Hepatic Adjustment
Reduce dose by 50% in severe impairment.

Avoid high single doses (pump & dump for 12-24h).

Contraindications
- First trimester of pregnancy (controversial)
- Alcohol consumption (disulfiram-like reaction)
Common Adverse Effects
- Metallic taste
- Disulfiram-like reaction with alcohol
- Peripheral neuropathy (prolonged use)
