Dosing & Administration
Adult Dose
4-8 mg PO/IV every 8 hours PRN.
Pediatric Dose
0.15 mg/kg IV (max 16 mg/dose).
Renal Adjustment
No adjustment necessary.
Hepatic Adjustment
Max 8 mg/day in severe impairment (Child-Pugh C).
