Dosing & Administration
Adult Dose
40 mg PO/IV daily. High-dose IV for GI bleed: 80 mg bolus then 8 mg/hr.
Pediatric Dose
0.5-1 mg/kg/day PO.
Renal Adjustment
No adjustment necessary.
Hepatic Adjustment
Consider max 20 mg/day in severe impairment.
