Dosing & Administration
Adult Dose
3.375g - 4.5g IV every 6 hours (often given as extended infusion over 4 hours).
Pediatric Dose
80-100 mg/kg of piperacillin component IV every 6-8 hours.
Renal Adjustment
CrCl 20-40: 2.25g every 6h. CrCl < 20: 2.25g every 8h.
Hepatic Adjustment
No adjustment necessary.
