Adult Dose
24/26 mg to 97/103 mg PO BID.
Pediatric Dose
Weight based dosing for pediatric heart failure.
Renal Adjustment
CrCl < 30: Start with lowest dose.
Hepatic Adjustment
Child-Pugh B: Start lowest dose. Child-Pugh C: Contraindicated.

- History of angioedema with ACEI/ARB
- Use within 36 hours of ACE inhibitor
