Dosing & Administration

Adult Dose

UTI/Skin: 1 DS tab (800/160 mg) PO BID.

Pediatric Dose

8-12 mg/kg/day (of TMP component) PO divided BID.

Renal Adjustment

CrCl 15-30: Reduce dose by 50%. CrCl < 15: Not recommended.

Hepatic Adjustment

Use caution.

Safety & Special Populations

Pregnancy Category

D - Avoid in 1st trimester and at term (kernicterus).

Lactation Safety

Avoid if nursing infant < 2 months old.

Contraindications & Adverse Effects

Contraindications

  • Sulfa allergy
  • Megaloblastic anemia due to folate deficiency

Common Adverse Effects

  • Stevens-Johnson Syndrome (SJS)
  • Hyperkalemia
  • Bone marrow suppression
