Clinical Significance

Determines the concentration of solutes in blood. Crucial for working up hyponatremia and evaluating for toxic alcohol ingestions (osmolar gap).

Causes of High

  • Dehydration
  • Diabetes Insipidus
  • Hyperglycemia
  • Uremia
  • Toxic alcohol ingestion (Methanol, Ethylene Glycol)

Causes of Low

  • Overhydration
  • SIADH
  • Adrenal insufficiency
