Dosing & Administration

Adult Dose

15-20 mg/kg IV every 8-12 hours (trough-based dosing). 125 mg PO QID for C. diff.

Pediatric Dose

15 mg/kg IV every 6 hours.

Renal Adjustment

Highly renally cleared. Dose strictly based on CrCl and trough levels.

Hepatic Adjustment

No adjustment necessary.

Safety & Special Populations

Pregnancy Category

C - Use only if clearly needed.

Lactation Safety

Compatible (poorly absorbed orally).
